Each form explains its purpose. We ask for the information needed to provide the selected resource or review your request.

Keep detailed records in the secure process

Do not enter Social Security numbers, medical records, detailed medical histories or another person's contact information into these forms. A family helper should provide their own details. Staff can explain the appropriate document process if a review proceeds.

Resource requests and subscriptions

Requesting a resource does not automatically subscribe you to a newsletter. Newsletter topics and permission are selected separately. A private management link controls an existing subscription request.

Optional qualification and source information

Review requests may ask about branch, retirement type, pay grade, service years, rating and prior application status. Leave optional answers blank or choose Not sure when unknown. We also retain the page path, campaign tags and referring website hostname to understand how requests reach us. Your own answer about where you heard of Standfast is stored separately.

How your request is handled

Your selected answers, contact details, source and consent are saved in a restricted Standfast request queue. Staff use them to provide the resource or review your request. Downloadable worksheets stay on your device; these forms do not upload their contents. Requesting a resource does not schedule an appointment or authorize newsletter enrollment.
