The August 2026 changes matter. DoD replaced the old JUL 2011 DD Form 2860 with a new AUG 2026 edition (it asks for your DoD ID number instead of your SSN and renumbers the disability section to item 14), and added a separate DD Form 3210 for reconsideration requests. If you downloaded the form before mid-August 2026, get the new one.

Your branch decides every CRSC claim: Army HRC, the Navy CRSC Board for Navy and Marine Corps, AFPC for Air Force and Space Force, and Coast Guard PSC. Where do I download DD Form 2860?

The current edition is dated AUG 2026. You can get it from the official DoD forms site or in the free Standfast CRSC forms kit on this page, along with the DD Form 3210 reconsideration request and the SF 180 records request.

Why does DD Form 2860 only show a "please wait" message?

The fillable edition is a dynamic (XFA) PDF. Save it to your computer and open it in free Adobe Acrobat Reader. Browser PDF viewers and most phone apps cannot display it. The print copy in the kit opens anywhere.

What is DD Form 3210?

DD Form 3210, dated August 2026, is the official CRSC reconsideration request. It is used when you were denied or want a decision reconsidered, and it should go in with new and substantive evidence.

Do I need an SSN on the new DD Form 2860?

The AUG 2026 edition asks for your DoD ID number, found on the back of your Common Access Card or retiree ID, instead of your Social Security number.
